TLA Member Assistance Request Form


At Tapestry LA, we believe the gospel brings renewal to every part of life—spiritual, relational, and practical. As a church family, we seek to walk together through seasons of hardship, carrying one another’s burdens in love. We understand that everyone faces difficult circumstances, and it takes humility and courage to ask for help.

We care deeply about your well-being and want to provide temporary financial assistance for members experiencing a crisis. Because these funds are supported by the generosity of our church, assistance is available only to active members who are meaningfully involved in the life and ministry of Tapestry LA.

If we are unable to meet your specific need, we will gladly connect you with our Restoration Center or other local resources that can provide ongoing support and care.

Purpose
The purpose of financial assistance is to help members experience God’s provision and restoration in times of crisis. This support offers short-term relief, not long-term financial aid. Each request is prayerfully reviewed as we seek to walk with you toward greater stability, healing, and stewardship.

Eligibility & Guidelines
To qualify, applicants must:
  • Be active members of Tapestry LA (regular attendance, participation in a Cell, and serving in ministry).
  • Be facing a verifiable short-term financial crisis (e.g., job loss, medical emergency, or unexpected hardship).
  • Be open to pastoral conversation, prayer, or financial coaching if recommended.
  • Provide supporting documentation (e.g., bills, statements, or invoices).
Types of Assistance (case by case):
  • Housing or rent support
  • Utilities (gas, water, electricity)
  • Medical emergencies
  • Essential living expenses
  • Groceries
Requests for non-essential items (e.g., car payments, travel, credit card debt, tuition) will be considered individually. Funds are disbursed directly to vendors or service providers, not to individuals (except for groceries). 

Application Process
  1. Complete the Member Assistance Request Form below.
  2. The Member Assistance Team will prayerfully review your request and respond within 5–6 business days.
  3. If approved, you may be invited to meet with a pastoral staff member for prayer, follow-up, or financial coaching.
Financial Counseling
To help members grow in financial wholeness, those requesting assistance may be asked to participate in a stewardship session with a pastor, elder, or executive team member. If you have received assistance more than once, counseling will be required before future requests are considered.

Benevolence Limits
To serve as many families as possible, assistance is typically limited to two requests per household per calendar year. The total amount of aid will depend on available resources and pastoral discretion.
